Status: agreed 2026-09-19 after a three-round design interview. This file is
the contract between the two repos, jev-lens (Claude Code hook plugin) and
jev-lens.nvim (editor side). Change it before changing either side.
1. Problem
Reviewing agent output is the bottleneck. lazydiff shows what changed. Nothing says whether you need to look at all, which files, or what kind of look. The habitual review is not line by line: it is "find the leftover comment blocks, check the overall shape". That is a handful of cheap judgments, so it should run automatically before a diff is ever opened.
2. Product in one paragraph
When a Claude Code session stops, a detached judge diffs the repo against a baseline (the tree as of the last review), asks Jev a batch of calibrated questions, and writes one verdict file per repo. Neovim watches that file. A green verdict is one quiet notification line. Anything else opens a popup: which files need attention, what kind, how sure, plus one-key actions: open lazydiff on the file, strip flagged debris, mark reviewed, jump to the session.
Gates (jev-gates) stop bad writes before they land. The lens ranks what did land for the human who signs off. They are complementary and independent.

Prompts keep the last 32. Edits keep the last 500. Writes are temp-file-plus-rename. Concurrent hooks may race; last writer wins, which loses at most one prompt or edit entry and never corrupts the file.

files is sorted by attention descending. kind is the top choice when
its confidence clears the bar, else "unsure"; kind_top always carries the
top choice so the popup can show it as a guess (out of scope?). Live data
showed a six-way choice rarely clears 0.7, so this field matters. debris
only holds candidates with p ≥ debris threshold, and no two candidates
claim the same line: a comment carrying a suppression or TODO marker is that
class alone. line numbers refer to the file as it was at tree; nvim
re-verifies lines content before deleting and refuses overlapping ranges.

All hooks: exit 0 always, no stdout, failures swallowed. Timeouts ≤ 5 s.
6. Judge (bin/judge.mjs)

- Resolve repo root. Not a git repo: active → exit silently; shadow → log.
- Load state. No baseline → snapshot now as baseline and exit (nothing to judge).
- If the working tree is clean and
HEADtree ≠ baseline: baseline := HEAD tree (reasoncommit-clean), delete verdict, exit. - Snapshot the working tree to a tree oid
tree(temp index,git add -A,git write-tree; respects.gitignore). - If
tree == baseline: delete any verdict, exit. Iftree == last_judged_treeand a verdict exists: exit (coalesce). git diff-tree -p --numstat baseline tree. Drop files matching the ignore list, and anything under the data dir if it lives inside the repo. Redact key-shaped strings. Clip each patch; clip total tomaxChars. Judge files in path order so positional question ids are stable.- Build call A (verdict) and call B (debris). Send. Validate.
- Compose verdict, sort, write atomically. Update
last_judged_tree. Log. - If
treechanged on disk during the run (re-snapshot and compare), markstale: trueso nvim shows the badge.
--reviewed: set baseline := current tree, reviewed_id := current verdict id,
mark verdict reviewed. nvim can do the same by editing state.json directly.

Default ignore: lockfiles (*lock*.json, *.lock, pnpm-lock.yaml,
yarn.lock, Cargo.lock, go.sum), dist/, build/, .next/,
node_modules/, *.min.*, *.map, *.snap, binary files, and the secret
family (.env*, *.pem, *.key, *.p12, *.pfx, id_*, .ssh/, .aws/,
.kube/, credentials*, secrets/).
Redaction: strings matching common key shapes (sk-…, ts_…, ghp_…, AWS
AKIA…, 32+ char base64/hex runs after key|token|secret|password and =
or :) are replaced with [redacted] before entering state.
7. Neovim plugin (jev-lens.nvim)
Zero dependencies. Neovim ≥ 0.10.

8. Non-goals for v1
Cross-session conflicts, hunk-level "why" from the edit log, Bash write attribution, sending corrections to the session, judging outside git repos, stripping anything without a confirm.
9. Edge cases and how they resolve
- Stop with no edits:
tree == baseline→ no verdict, pending cleared. - Stop re-entered (
stop_hook_active): return without spawning. - Two stops in a row: same tree → coalesce; different tree → later run overwrites, earlier run notices the tree moved and marks itself stale.
- Human edits between stops: judged as part of the diff. The kind question does not attribute; the edit log will in v1.5.
- Lockfile churn: ignored by default; listed in
summary.skipped. - Huge diff: files over the cap are listed with
attention: nullandkind: "unjudged"so the popup shows them as unreviewed, never as safe. - Renames, deletions, binaries: renames judged as modify; deletions get a patch of removed lines; binaries skipped and listed.
- Buffer modified when stripping: refused with a message naming the file.
- Line moved since judged: content check fails, that candidate is skipped and counted as "skipped, content moved".
- No nvim open: verdict persists; shown once on next nvim start in the repo.
- Several nvims in the repo: all render; dismiss is local; reviewed is shared via state.json.
- Worktrees: key is the worktree root, so each worktree has its own baseline.
- No API key / API down: judge logs
no-keyor the error and writes no verdict. nvim shows nothing. Silence is never green.
